FERPA Form
This form was developed for use by all students in CECAS. This form must be completed by any student for whom you are sharing information about. Once completed, this form should be uploaded to the student’s record in CU Navigate for undergraduate students. If you do not have access to a specific student’s record in CU Navigate, contact your department’s student services staff member or the students advisor. Forms for graduate students shall be maintained using an alternate method as CU Navigate has not been implemented for graduate students.
