Human Resources has mailed IRS form 1095-C to eligible employees

Eligible employees can expect to receive their IRS form 1095-C for the 2015 tax year no later than March 31, 2016. The form will be delivered by the U.S. Postal Service to the home address. Employees can also access their form online through HR Self Service (Self Service>Benefits>View 1095-C).

The 1095-C, also known as the Employer-Provided Health Insurance Offer and Coverage statement, contains important information about medical coverage offered to employees and their dependents by Clemson University. The information on Form 1095-C may assist in tax preparation, but the form is not required for filing.

In compliance with Affordable Care Act reporting requirements, the Office of Human Resources (OHR) mailed the form to all employees who were eligible to participate in state health insurance through Clemson University at any time in 2015. Clemson University is also required to provide a copy of individual forms to the IRS. Employees who were not eligible for state health insurance through their Clemson employment in 2015 will not receive a form.
